Insurance and Billing
Surgery charges at Casa Colina Surgery Center will consist of a base facility fee and anesthesia fee. We will require you to pay any deductible or coinsurance on the day of surgery. We will check with your insurance to find out this total for you, which will be part of the facility fee. The anesthesia bill will come from the anesthesiologist’s office.
Prior to your surgery, estimation for this charge can be provided for you. Additional charges may be necessary for time over the estimate we give you.
Before your surgery date, you will be notified of your insurance coverage and any deductible or copay due on the day of your surgery. Should you have specific questions regarding your plan, we suggest that you call your insurance company. We will file your insurance papers for you. For your convenience, we accept Visa, MasterCard and American Express for your deductible or copay. We also offer the option to pay bills online. With our online bill pay option, you can securely make a payment for your service online with no hassles.
Pay Bill OnlineShould the facility bill or anesthesia costs exceed what is anticipated; you will be billed for the difference. Overpayments will be refunded, at the latest, 30 days from receipt of your insurance’s payment. Our facility bill will not include the services of your surgeon, anesthesiologist or any other outside services.
You have the right as a patient to request information and have your questions answered regarding any fees or payment policies. If you have any questions, please don’t hesitate to call our business office at (909) 593-3550. Please call within the hours of 8 a.m. and 5 p.m., Monday through Friday.
Uninsured Procedures
Facility Fees for uninsured procedures will be collected prior to your surgery. For your convenience we accept Cash, Visa, MasterCard, American Express or money orders. Anesthesia Fees need to be paid directly to their office the day prior to surgery. Anesthesia charges are based on actual time used and will be billed by the anesthesiologist. Because you are responsible for costs for covered healthcare services until you reach your deductible, you should comparison shop if you elect to have a nonemergency procedure.
